The practice has a process and materials that it provides patients/families on the role of the medical home, which include the following:

  • The practice is responsible for coordinating patient care across multiple settings
  • Instructions on obtaining care and clinical advice during office hours and when the office is closed
  • The practice functions most effectively as a medical home if patients/families provide a complete medical history and information about care obtained outside the practice
  • The care team gives the patient/family access to evidence-based care and self-management support

As you organize the above material, know that documentation about practice medical home responsibilities can be generated automatically by Partner based on the visit reason. For example, if a patient is coming in for an ADHD visit, Partner can automatically generate special, PCMH-relevant documentation to give to this patient. Partner can even be programmed to print this documentation for only the first visit every year.

This information can also be set up to print manually by the front office at the time of patient check-in.

For practices that use EHR, providers can "order" these materials to be generated and printed for patients when necessary.
