Assesses the racial and ethnic diversity of its population

Race and ethnicity are tracked in Partner in the patient editor and can easily be updated at the time of checkin. This information can also be updated in PCC EHR. Partner reports can be generated to show how many patients have each race and ethnicity value.

Assesses the language needs of its population

Preferred language is tracked in Partner in the patient editor and can now easily be updated at the time of checkin. This information can also be updated in PCC EHR.

The recaller output and spreadsheet described above also includes an assessment of a patient's primary preferred language.

Provides interpretation or bilingual services to meet the language needs of its population

The practice would need to provide documentation showing their use of interpretation services. When the practice uses staff or an outside service to interpret during a visit, this should be noted in the visit.

Provides printed materials in the languages of its population

Expecting patients/families to select a personal clinician
