The practice uses a searchable electronic system and records data more than 50% of the time for the following:

  • Date of birth
  • Gender
  • Race
  • Ethnicity
  • Preferred language
  • Telephone numbers
  • E-mail address
  • Dates of previous clinical visits
  • Legal guardian/health care proxy
  • Primary caregiver
  • Presence of advance directives (N/A for pediatrics)
  • Health insurance information

To achieve all points for this element, you must meet 50% for 9-12 of the above factors. All of the above patient information can be tracked in Partner. "Presence of advance directives" does not apply for pediatric practices, so you will get credit for that one. "Primary Caregiver" is something that you probably don't track and it's implied that this is supposed to be a separate field from "Legal Guardian" (Custodian). For race and ethnicity, if the patient "prefers not to answer" specify that and you would get credit. You would need to produce a report showing the percentage of all patients seen in a three month date range that have the above fields documented.

A report with this information can be generated through recaller. Instructions are listed below, but please contact PCC support if you need assistance.

Create a form letter with these fields and add it to /dat/forms/rec_forms:

Generate a patient list in recaller restricted to patients whose last visit was in a desired three month date range, excluding patients with certain "inactive" flags. Send the output to the form letter above and email it to yourself. Add a .csv extension to the file that is emailed to you and then open it into a spreadsheet.

PCC has created the attached spreadsheet template that can be used to calculate the percentages of patients with each field. Simply paste in the data into the "data" sheet starting at row 5. THe percentages will automatically be calculated and displayed on the other sheet. A screenshot of the final summary table is also attached. Spreadsheet template
