The practice has a process and materials that it provides patients/families on the role of the medical home, which include the items below.
As you organize the material, know that documentation about practice medical home responsibilities can be generated automatically by Partner based on the visit reason. For example, if a patient is coming in for an ADHD visit, Partner can automatically generate special, PCMH-relevant documentation to give to this patient. Partner can even be programmed to print this documentation on a periodic basis (i.e., for only the first visit every year). This information can also be set up to print manually by the front office at the time of patient check-in.
For practices that use EHR, providers can additionally "order" these materials to be generated and printed for patients when necessary.
