More than 50 percent of patients who request an electronic copy of their health information (e.g., problem list, diagnoses, diagnostic test results, medication lists, allergies) receive it within three business days
This is a Meaningful Use measure and reportable from the EHR. PCC EHR can generate a PDF with this information which can be mailed or saved to CD/memory stick per ARRA requirements. Partner users can generate a problem list, diagnosis history, and allergy list and then use eRX to create a medication list. We don't currently have a way to share this information electronically via a patient portal, although it can be emailed from the EHR.
At least 10 percent of patients have electronic access to their current health information (including lab results, problem list, medication lists and allergies) within four business days of when the information is available to the practice
Until we add the ability to share health information via the PCC Patient Portal and can count how many patients have access to their information, PCC clients won't be able to meet this one.
Clinical summaries are provided to patients for more than 50 percent of office visits within three business days
This is a Meaningful Use measure and reportable from the EHR. PCC EHR can print such a copy of health information. The practice could print this for a patient after the visit has been documented.

Any PCC practice with a WWW site can technically do this now with a form that allows the patient to email the practice. A patient portal is the optimal communication method.
