3.B Clinical Data

The practice uses an electronic system with the functionality in factors 6 and 7 and records the information in factors 1–5 and 8–11 as structured (searchable) data:

3B.1 An up-to-date problem list with current and active diagnoses for more than 80 percent of patients

Refer to the "Problem List" measure in PCC's stage 1 Meaningful Use report.

3B.2 Allergies, including medication allergies and adverse reactions,* for more than 80 percent of patients

Refer to the "Medication Allergy List" measure in PCC's stage 1 Meaningful Use report.

3B.3 Blood pressure, with the date of update, for more than 80 percent of patients 3 years and older.

Refer to the "Vital Signs (Weight/Height Exception)" measure in PCC's stage 2 MU report. This measure version excludes weight and height and looks at patients > 3 years old who had just BP recorded.

3B.4 Height/length for more than 80 percent of patients.

3B.5 Weight for more than 80 percent of patients

For 3B.4 and 3B.5, you can't refer to the MU report since there isn't one that reports just height/length and just weight recorded. To get the calculation you'll need, you'll want to run a combination of the MU report to get the denominator and an EHR "Patient Lists" report to get the numerator. For the denominator, you can use the denominator reported on the MU report for "Vital Signs (BP Exception)" since this will include all patients seen regardless of age. For the numerator, create two patient lists: "Patients with height/length recorded" and "Patients with weight recorded", making sure to include patients with height/length and weight > 0. Run these reports to identify patients to count in the numerator for 3B.4 (height/length recorded) and 3B.5 (weight recorded).

3B.6 System calculates and displays BMI

If you use PCC EHR, you can get auto credit for this since PCC automatically calculates and displays BMI

3B.7 System plots and displays growth charts (length/height, weight and head circumference) and BMI percentile (0-20 years) (NA for adult practices)

3B.8 Status of tobacco use for patients 13 years and older for more than 80 percent of patients

3B.9 List of prescription medications with date of updates for more than 80 percent of patients

For 3B.8 (Smoking Status) and 3B.9 (Medication List) refer to the stage 1 MU report output for your % for those factors.

3B.10 More than 20 percent of patients have family history recorded as structured data

3B.11 At least one electronic progress note created, edited and signed by an eligible professional for more than 30 percent of patients with at least one office visit

For 3B.10 (Family health history) and 3B.11 (Electronic Notes) refer to the stage 2 MU report output for your % for those factors.
