5.A.6 Follows up with the inpatient facility about newborn hearing and newborn blood-spot screening (NA for adults).

NCQA will require a documented process and evidence showing how the process is met, such as a report or a log or examples (to receive credit for the factor the practice must show evidence across patients not just a single example).

5.A.7 More than 30 percent of laboratory orders are electronically recorded in the patient record.

Refer to the "CPOE Laboratory" measure results in the "Stage 2" or "Modified Stage 2" version of the Meaningful Use report in PCC EHR. Since all lab orders are made and recorded electronically in PCC EHR, you will always report 100% for this measure.

5.A.8 More than 30 percent of radiology orders are electronically recorded in the patient record.

Refer to the "CPOE Radiology" measure results in the "Stage 2" or "Modified Stage 2" version of the Meaningful Use report in PCC EHR. Since all radiology orders are made and recorded electronically in PCC EHR, you will always report 100% for this measure.

5.A.9 Electronically incorporates more than 55 percent of all clinical lab test results into structured fields in medical record.

Refer to the "Lab test Results" measure results in the "Stage 2" version of the Meaningful Use report in PCC EHR. This measure tracks individual lab tests, not lab orders. Use the lab configuration tool to configure discrete lab tests for all lab orders. Any e-lab results received automatically count if positive, negative, or numeric.

5.A.10 More than 10 percent of scans and tests that result in an image are accessible electronically.

Refer to the "Imaging Results" measure results in the "Stage 2" version of the Meaningful Use report in PCC EHR. To meet this measure, discrete radiology orders will need to have scanned images attached to them.
